Professionals & Referral Partners
A responsive, coordinated pathway to behavioral health and addiction treatment for the patients and clients you serve.
Prosperous Health partners with hospitals, physicians, behavioral health clinicians, case managers, health plans and community professionals to help individuals access appropriate addiction and mental health treatment.
Our admissions and clinical teams provide a coordinated point of contact for reviewing referrals, identifying available services and supporting transitions across the continuum of care.
Placement recommendations and admission decisions are based on each person’s clinical needs, safety considerations, program availability, eligibility requirements and applicable payer authorization.
A Connected Referral Pathway
Navigating behavioral health treatment can be complex—particularly when a patient is transitioning from a hospital, emergency department, outpatient practice or another treatment program.
Prosperous Health helps referral partners coordinate access across a network of behavioral health and addiction-treatment services. Our goal is to make the referral process clear, responsive and collaborative while protecting patient privacy and supporting continuity of care.

How the Referral Process Works
1. Contact Our Admissions Team
Call (888) 207-3422 to discuss the individual’s needs, requested level of care, anticipated discharge timeline and other relevant circumstances.
2. Share Information Securely
Our admissions team will explain what information is needed and provide an approved method for securely transmitting clinical or protected health information.
Do not send protected health information, medical records or other confidential patient information through standard email or an unsecured website form.
3. Clinical Review
Qualified professionals review the available information to evaluate clinical appropriateness, safety considerations and potential treatment options. Additional documentation or consultation may be requested.
4. Benefits and Authorization Coordination
When treatment will involve insurance coverage, the appropriate team may verify available benefits and coordinate applicable authorization requirements. Benefit verification does not guarantee coverage or payment.
5. Program and Facility Matching
When appropriate, the individual is matched with an available facility, program or level of care based on clinical needs, eligibility, location, capacity and payer requirements.
6. Transition Coordination
Our team works with the referring professional, patient, family or authorized representative to coordinate the next steps. Transportation and transition assistance may be available depending on the facility, location and circumstances.
7. Continuing Collaboration
With appropriate patient authorization, we support communication with referral partners during transitions and discharge planning to promote continuity of care.

When available and legally authorized for disclosure, the following information may assist the review process:

- Referring professional and organization
- Presenting concerns and reason for referral
- Requested or recommended level of care
- Substance-use history and date of last use, when applicable

The admissions team will determine which records are necessary and how they should be transmitted securely.
Hospital & Emergency Department Coordination
Prosperous Health works with hospital and emergency department teams to evaluate potential behavioral health and addiction-treatment referrals following appropriate medical screening and stabilization.
Depending on the patient’s condition and available programs, our team may assist with:

- Identification of an appropriate treatment setting
- Coordination with physicians, nurses and case managers
- Benefit and authorization review
- Transfer and transportation planning
- Communication with authorized family members
- Step-down and continuing-care planning
A referral or preliminary review does not guarantee admission. Final acceptance depends on clinical appropriateness, medical stability, program criteria, available capacity and applicable payer requirements.
