Physician-Led Addiction Medicine

Prosperous Medical provides physician-led addiction medicine and coordinated medical and psychiatric services across the outpatient continuum. Led by Matin Hemmat, MD, FASAM, a physician board-certified in Addiction Medicine and Internal Medicine, our approach combines careful assessment, individualized treatment planning, medication options and close coordination with behavioral health providers.

Clinical Services

Addiction Medicine Evaluation & Treatment Planning

Comprehensive physician evaluation of substance use, withdrawal risk, medical history, medications, co-occurring conditions and recovery goals, followed by an individualized recommendation for care.

Ambulatory Withdrawal Management

Physician-directed withdrawal management delivered in an outpatient setting for carefully selected patients whose clinical needs can be managed safely without inpatient hospitalization.

In-Home & Hybrid Withdrawal Management

A combination of in-person and telehealth-supported medical care that may be available to appropriately screened patients in the privacy of their home. Eligibility depends on medical risk, withdrawal history, the home environment, available support and the patient’s ability to participate safely in treatment.

Medications for Addiction Treatment

Evaluation and management of FDA-approved medications for alcohol and opioid use disorders, which may include buprenorphine, naltrexone, acamprosate and disulfiram when clinically appropriate. Patients who may benefit from methadone are referred to a certified opioid treatment program.

Psychiatric Collaboration & Medication Management

Medication evaluation and ongoing support for co-occurring psychiatric conditions through appropriately qualified providers, coordinated with the patient’s addiction treatment plan.

Co-Occurring Disorder Care

Integrated planning for patients experiencing substance use and mental health conditions together, with communication among medical, psychiatric and behavioral health professionals.

Co-Occurring Medical Conditions

Coordination and medical management for conditions that may affect treatment, including diabetes, hypertension and chronic pain, when clinically appropriate and within the treating provider’s scope. Referrals are arranged when specialty care is needed.

Clinical Coordination Across Levels of Care

Medical recommendations and continuity coordinated with the appropriate outpatient, intensive outpatient, partial hospitalization, residential or hospital setting according to each patient’s evolving clinical needs.

Ambulatory Withdrawal Management With Safety at the Center

Some patients can receive withdrawal management safely in an outpatient or home-based setting, while others require residential or hospital-level care. The appropriate setting can only be determined after an individualized medical evaluation.

A physician considers the substances involved, expected withdrawal severity, previous withdrawal complications, medical and psychiatric conditions, current medications, available support and the safety of the proposed treatment environment.

When ambulatory care is appropriate, treatment may include scheduled medical follow-up, symptom monitoring, medication management, hydration and nutrition guidance, behavioral support and ongoing reassessment. If risk increases or a higher level of care becomes necessary, the treatment plan is adjusted and the patient is transitioned to an appropriate setting.

What to Expect

  1. A patient, family member or healthcare professional submits a referral.
  2. A physician completes a comprehensive assessment of the patient’s medical, psychiatric and substance-use needs.
  3. The physician recommends an appropriate treatment setting and develops an individualized medical plan.
  4. Care begins in the recommended setting setting, with ongoing reassessment and coordination throughout treatment.
